Therapy sessions:

 

Occupational therapy sessions consist of:

- One 90 minute initial assessment and goal setting session via video Telehealth (75 minutes face to face, 15 minutes therapist documentation time).

- 60 minute therapy sessions, either in your home (travel charges apply) or via video Telehealth.

- 30 minute therapy reviews/ follow up sessions as applicable

 

Fees:

 

Occupational therapy time is billed at $193.99 per hour, in line with current NDIS guidelines.

 

- Initial assessment (75 minute session, 15 minute therapist documentation time): $290.98

- Therapy sessions (60 minutes face to face, documentation time not charged): $193.99.

​- ​Travel time for home ​and community sessions is charged in 15 minute intervals, at $193.99 per ​hour.

- All formal report writing, funding applications, telephone calls, and follow up that exceeds 15 minutes are charged at a rate of $193.99 per hour.

 

Cancellation policy:

 

- A minimum of 24 hours notice is required for all appointment cancellations, otherwise the full fee will be charged.

 

Illness:

 

- Please inform your occupational therapist prior to your appointment if you are unwell.

- You may be required to wear a mask during your therapy session.

- Where possible, a Telehealth appointment will be offered in place of your face to face session.

- In order to provide a service to you, Identity Occupational Therapy collects and uses your private information. Your information is collected and stored in line with the Privacy Act of 1988 and Occupational Therapy Australia's Code of Ethics. Your privacy is respected, and information about you will not be shared with others, unless required by law (e.g in court, legal practitioners, regulatory authorities), or consented to/ requested by you (e.g other therapists and medical professionals).Should you choose to withhold information or provide inaccurate information, you need to be made aware that this will impact the therapy provided to you by Identity Occupational Therapy.
